- A death certificate in the name of the deceased 
- An official, valid, and enforceable instrument in the form of a Last Will and Testament or other Probate documents 
- Transfer instructions signed by the beneficiary, authority, or entity legally representing the rights of the deceased and their estate 
- Government-issued photo identification of the beneficiary
